The following forms will be required before we can begin training together and we will go over these completed forms during your initial consultation!

If you are a first-time client, please print this form, complete it, and bring it along to our first session.
Intended Outcomes Form (optional, but strongly recommended)
This form will help you clarify your goals for your overall health and vitality, including our time together.
PAR-Q (The Physical Activity Readiness Questionnaire) *
This questionnaire will tell you whether it is necessary for you to seek further advice from your doctor or a qualified exercise professional before becoming more physically active.
Medical History Questionnaire *
This form is used to indicate whether or not you have ever been diagnosed with that particular condition.
This form should be used only if you have a medical condition that will prohibit you from starting a workout regiment and need clearance from your doctor first.
Exercise History Questionnaire *
This form is used to establish your experience with different exercises; sports; and/or extracurricular activity in the past.
Three-Day Dietary Record (optional, but strongly reccomended)
This form is used to record your dietary intake. Basically writing down EVERYTHING that you consume. That includes sauces, oil/butter used to cook with, herbs/spices, etc
This form is used to give consent to participate in a physical fitness program
This form is used as an agreement between client/trainer that any and all information exchanged between the two individuals will NOT be shared with anyone without written authorization by the client.
